| This RMA Request Form is not an
authorization to return any product. This form is used only as a request
to obtain an authorized RMA number. An authorized RMA number must be
issued prior to shipping any products back. All Daly Computers RMA
numbers are valid for 10 days. Products received after 15 days or with
unreported physical damage to the product(s) will be refused. In
addition, Daly Computers reserves the right to refuse any product
returns that differ from the information contained on this form.
Over/under shipments must be reported immediately. You will be contacted
by a Daly representative within 24 hours of submitting this form (not
counting weekends). You can either complete this form online or by using
the downloadable form, fax it back to (301) 556-4243 or email the form
to
